There is no need to call each month to request your order. Re-focus the energy of ordering supplies back into the care of yourself or loved one.
Once we partner together on your care, you are invited to be an active participant through our Brightree Patient Hub smartphone app. You will have 24/7 access to your profile, insurance information, and order status. You will automatically receive an email confirmation every time your order ships or is delivered by one of our drivers.

If you are ready to begin services:
- Complete the information by clicking “Submit a Product Request” below according to your needs or needs prescribed by your doctor
- If you already have a prescription, email it to firstname.lastname@example.org
- Our staff will confirm insurance eligibility and request a prescription and other required documentation from your physician
- We will confirm your order with you via phone, text, or email before shipping
- Your insurance benefit will ultimately determine the quantity of product allowed per 31 day cycle
- We will notify you of any copays or out-of-pocket expenses before your order ships
